Fatigue through the menopause transition

Fatigue during perimenopause often comes from overlapping hormone variability and interrupted sleep. Estrogen fluctuates across the transition instead of declining along a smooth line. Night sweats fragment sleep even when the total time in bed looks adequate. Daytime fatigue then reflects lost sleep alongside hormone variability. The timing makes the fatigue less predictable from one week to the next.

Iron and thyroid, the two most missed causes

Iron deficiency and thyroid disease deserve separate testing when fatigue has no clear explanation. Heavy menstrual bleeding often reduces iron stores before hemoglobin falls below a laboratory reference range. A normal complete blood count therefore does not rule out iron deficiency. Thyroid disease is much more common in women. The American Thyroid Association reports that women are five to eight times more likely than men to develop thyroid problems.
