Foundayo is the brand name for orforglipron, a once-daily weight-loss pill from Eli Lilly. It is a small-molecule, non-peptide glucagon-like peptide-1 (GLP-1) receptor agonist. The U.S. Food and Drug Administration (FDA) approved it on April 1, 2026, to reduce excess body weight and maintain weight reduction long term.1,2 In ATTAIN-1, the highest dose studied produced an average 11.2% body-weight reduction at week 72, compared with 2.1% for placebo among adults with obesity without diabetes.3
Orforglipron and the GLP-1 Receptor
Orforglipron is a small-molecule oral medication that binds to and activates the GLP-1 receptor. GLP-1 helps regulate appetite and caloric intake, which can reduce how much food a person consumes.1
Its non-peptide structure places orforglipron in a different chemical class from peptide GLP-1 medicines. Foundayo is taken as a pill, which gives your physician another route to consider when injections do not fit your preferences.

ATTAIN-1 Results at 72 Weeks
ATTAIN-1 tested once-daily orforglipron in 3,127 adults with obesity who did not have diabetes. The multinational phase 3 trial compared three doses with placebo for 72 weeks alongside diet and physical activity.3
Average body-weight change differed by dose. Participants lost 7.5% with the lowest dose studied, 8.4% with the middle dose, and 11.2% with the highest dose. The placebo group lost 2.1% on average.3
Among participants assigned to the highest dose, 54.6% lost at least 10% of their starting weight. Another 36.0% lost at least 15%, and 18.4% lost at least 20%. The corresponding placebo rates were 12.9%, 5.9%, and 2.8%.3
Waist circumference, systolic blood pressure, triglycerides, and non-high-density lipoprotein cholesterol improved compared with placebo.3 Adverse events led 5.3% to 10.3% of orforglipron participants to stop treatment, compared with 2.7% of the placebo group. ATTAIN-1 was funded by Lilly.3
These figures describe averages across trial groups. They do not predict what you will lose or how quickly your weight will change. Your response has to be measured over time.
The Label’s Indication and Daily Use
The Foundayo label covers adults with obesity and adults with overweight who have at least one weight-related medical condition. It is indicated alongside a reduced-calorie diet and increased physical activity to reduce excess body weight and maintain the reduction over time.1
The label does not recommend combining Foundayo with another GLP-1 receptor agonist.1 If you already use a GLP-1 medication, your prescriber needs the exact drug and formulation before considering any change.
The label requires once-daily oral use, with or without food, and requires the tablet to be swallowed whole. It says the tablet should not be broken, crushed, or chewed.1 Your prescriber or pharmacist explains how those requirements apply to your medication schedule.
Lilly describes Foundayo as a pill that can be taken at any time of day without food or water restrictions.2 The label also includes stepwise dose escalation to reduce gastrointestinal reactions. Your prescriber sets that escalation based on the label and your tolerance.1
A standard escalation does not replace follow-up. Your tolerance, labs, and weight trend guide changes to the broader plan.
Foundayo Side Effects and Physician Follow-Up
Gastrointestinal (GI) symptoms were the most common Foundayo side effects in the label’s safety pool. That pool included 3,155 adults from two placebo-controlled trials followed for up to 72 weeks.1
The label reports these rates across the three maintenance strengths studied.1

Nausea, vomiting, and diarrhea were more frequent during dose escalation and decreased over time. Across the label’s pooled trials, 8% of participants taking Foundayo permanently stopped because of adverse reactions, compared with 3% taking placebo.1 Your prescriber can adjust the treatment plan when symptoms affect eating, hydration, or your ability to stay on therapy.
The boxed warning states that orforglipron is not pharmacologically active in rats or mice and did not produce tumors in rodents. It also states that the human relevance of GLP-1 receptor-dependent thyroid C-cell tumors observed in rodents has not been determined.1 Foundayo is contraindicated with a personal or family history of medullary thyroid carcinoma (MTC), with Multiple Endocrine Neoplasia syndrome type 2 (MEN 2), or with known serious hypersensitivity to orforglipron.1
Foundayo delays gastric emptying, which can affect how other oral medicines are absorbed.1 If you take prescription pills or supplements, your physician and pharmacist need the complete list before Foundayo is added.
Physician Review Before and During Treatment
A physician decides whether Foundayo fits by comparing the label with your medical history, medications, symptoms, and weight-related goals. Your diagnosis must fit the approved indication, and your history must be checked against the label’s contraindications.
Your medication list receives separate attention. Another GLP-1 receptor agonist creates a label limitation, while delayed gastric emptying can affect other oral medicines. Your physician may need to coordinate timing or monitoring with your pharmacist.1
Treatment preferences also count. You may prefer a daily pill, or you may find a weekly injection easier to follow. The route only helps when you can use the medication consistently under the prescribed plan.
After treatment begins, your prescriber sets the escalation and tracks nausea, constipation, diarrhea, vomiting, and other reactions. Your weight trend also needs context. One week is a data point. Repeated measurements show whether the direction is holding.
