Estradiol Levels in Men and What They Mean

Estradiol levels in men must be read with the lab's units and assay. In 64 men, a traceable liquid chromatography tandem mass spectrometry method produced a population reference interval of 12 to 136 pmol/L.1 This interval is not a treatment target or an age-specific chart. In a separate trial, estrogen deficiency primarily accounted for increases in body fat, while estrogen and androgen deficiency both contributed to reduced sexual function.2 Your physician should interpret the result beside testosterone, symptoms, and repeat measurements.

Estradiol's Function in Men

Estradiol contributes to body composition and sexual function in men. Testosterone is converted to estradiol, which means both hormones can contribute to symptoms when gonadal hormone production is suppressed.2

Finkelstein et al. studied two cohorts of healthy men aged 20 to 50 after pharmacologic suppression of endogenous testosterone and estradiol. The first cohort included 198 men assigned to placebo or different amounts of testosterone gel. The second included 202 men who also received anastrozole to suppress the conversion of testosterone to estradiol.2

Finkelstein and colleagues concluded:

> "Androgen deficiency accounted for decreases in lean mass, muscle size, and strength; estrogen deficiency primarily accounted for increases in body fat; and both contributed to the decline in sexual function."2

The trial examined hormone deficiency under controlled conditions. It did not study elevated estradiol or define an ideal estradiol level for men on testosterone therapy.

Reference Intervals for Estradiol Levels in Men

A male estradiol reference interval depends on the measurement method. Verdonk et al. used liquid chromatography tandem mass spectrometry (LC-MS/MS) to measure serum estradiol in 64 men.1

The researchers compared the method with the Centers for Disease Control and Prevention (CDC) reference method and reported that the LC-MS/MS method was traceable to it.1

The comparison rows show why a general estradiol chart can mislead you. Ranges developed for another population or physiological state do not answer the same question as a male reference interval.

The male interval is also method-specific. It describes a population measured with a traceable LC-MS/MS method. It does not establish an optimal level, a treatment threshold, or a level that guarantees the absence of symptoms.

Normal Estradiol Levels by Age

The traceable male reference interval is not divided by age. Verdonk et al. reported one interval for 64 men and did not publish separate male ranges by decade.1

An age-stratified male chart cannot be built from this study. Your physician can account for your age during the wider clinical review, but age does not create a supported cutoff within these data. The male interval printed by your laboratory should identify the method used for your test.

Estradiol Units and Assay Methods

An estradiol number has little clinical use without its units and assay method. Verdonk et al. reported results in pmol/L, while many laboratories in the United States report estradiol in pg/mL.

A result listed simply as "45" is incomplete. A value of 45 pmol/L and a value of 45 pg/mL use different units, so the numbers cannot be compared directly. No conversion should be performed without a validated conversion source.

Assay means the laboratory method used to measure estradiol. The reference interval in the table applies to traceable LC-MS/MS methods, according to Verdonk et al.1 A result from another method is not automatically comparable to that interval.

A complete interpretation uses four details:

  • Units, which show how the result is expressed.
  • Assay method, which identifies how estradiol was measured.
  • Laboratory reference interval, which provides the comparison range for that method.
  • Related hormone results, which show how estradiol fits beside total and free testosterone.

Keep the full laboratory report available during your physician review. The number, units, method, and related hormones belong in the same conversation.

Estradiol During Testosterone Replacement Therapy

Testosterone provides the substrate used to produce estradiol. Raising testosterone therefore increases the amount of substrate available for that conversion.2

This mechanism matters during testosterone replacement therapy (TRT), but the Finkelstein trial was not a study of ordinary TRT care. It studied healthy men aged 20 to 50 after pharmacologic suppression of gonadal hormones.2 The findings establish physiological functions for estradiol. They do not define high estrogen on TRT symptoms or a treatment cutoff.

The Endocrine Society guideline recommends diagnosing hypogonadism only when symptoms and signs occur with unequivocally and consistently low testosterone concentrations.3 It calls for an accurate fasting morning total testosterone test and confirmation with a repeated measurement.3

When total testosterone is near the lower limit or sex hormone-binding globulin affects interpretation, the guideline recommends obtaining free testosterone through equilibrium dialysis or an accurate formula.3 Your testosterone blood test gives estradiol the related hormone context it needs.

Clinical Context for a High Estradiol Result

A high estradiol result does not confirm a specific male syndrome. The cited studies do not establish a validated checklist of high estrogen in men signs or a single estradiol level that requires treatment.

Finkelstein et al. studied hormone deficiency. The trial did not establish symptoms caused by elevated estradiol.2 Verdonk et al. established a method-specific population interval. The study did not define a concerning threshold or treatment target.1

Symptoms still belong in the evaluation because hormone decisions are made from a pattern. Your physician can review the assay, units, laboratory interval, total testosterone, free testosterone, symptoms, and results from more than one draw.
