High Testosterone in Men and What It Means

Signs of high testosterone cannot confirm that your level is elevated. In an adult man, an elevated testosterone result is a lab finding that a physician needs to confirm and explain. The guideline recommends free testosterone testing when total testosterone is near the lower limit or a condition alters sex hormone-binding globulin (SHBG). SHBG is a lab marker used when interpreting total testosterone.1 That recommendation applies to deficiency evaluation, so it should not be presented as a standard workup for an elevated result.

Your physician confirms the number, reviews the test conditions, and then establishes why the result is unexpected. Treatment decisions should follow that evaluation.

Lab Results Over Physical Signs

No symptom or physical feature has been validated as an indicator of elevated testosterone in men. A popular sign list cannot tell you what your blood contains.

That limitation also applies to observations about appearance, personality, smell, sexual function, and morning erections. These observations may prompt you to ask about testosterone, but they cannot confirm an elevated concentration. Measurement provides the answer.

Exogenous Androgens and Gonadotropin Suppression

Christou et al. conducted a systematic review and meta-analysis covering 33 studies and 3,879 participants. The studies included three randomized clinical trials, 11 cohort studies, 18 cross-sectional studies, and one non-randomized parallel clinical trial.2 Most participants were men.

Anabolic-androgenic steroids are testosterone derivatives used to improve athletic performance or appearance. The anabolic-steroid literature reports potentially serious and irreversible adverse effects on the reproductive system.2 It does not provide a symptom checklist or a complete workup for every elevated testosterone result.

The trial did not study men with pathologically elevated testosterone. It examined healthy men with experimentally suppressed gonadal function. Its useful finding is the wide variation in how much testosterone men required to maintain the measured outcomes.3
